Report a Claim

There are 2 ways to report claims to Gallagher Bassett Services: via Internet or Telephone.

Internet: Mountainview: In order to access the Mountainview system, you will need a Risx-Facs ID and Password. Contact our Smyrna office at 1-800-582-5086 if you do not have one already, or if you need some training on the Mountainview software program. For training on Mountainview, click on the following Word Document for instructions.

Be sure that you have the following information ready when you report an injury:

The Gallagher Bassett Client ID 004118.
Name of the worksite location where the injury occurred
Date, Time, and Place of accident
Name and Social Security number of injured worker
NCCI Code of the injured worker
Description of the accident and if a 3rd party was involved
Was this a catastrophic accident or did a death occur
Were any authorities contacted or involved
Was the injured employee drug tested?

Reminder: In the event of death, catastrophic injury, amputation or an accident that involved 3 or more persons you must contact OSHA (800) 321-6742 or in California, CALOSHA (877) 922-7233. NOTE: Some states have different requirements. Contact your appropriate state agency for additional reportable claims.

Once the claim has been reported you will receive an electronic copy within 24 hours.

Forms for First Script

Gallagher Bassett offers a comprehensive risk management claims platform through Risx-Facs. This product does have user fees. For detailed information, User ID's, and passwords contact your Artex Analytics Advocate at 800-582-5086.
