Artex Cedar Hill offers its Middle Market PEO and Staffing Services customers a variety of on-line tools to facilitate management of the claims process.
Report a Claim
As of 1/1/11 all claims occurring on or after 1/1/11 must be reported to Gallagher Bassett Services, Inc., regardless of policy effective dates. All claims occurring prior to 1/1/11 must be reported to Broadspire. The Toll Free Number to report claims will remain the same, (877) 94-Cedar (877) 942-3327 for both Broadspire and Gallagher Bassett. Please let the representative, taking the report, know to forward the claim to Broadspire if it occurs prior to 1/1/11.

Produce a Loss Run
Loss runs can be produced by accessing our Client Services on-line program. If you have not obtained a user ID and password for this program, or do not have the Loss Runs option activated, contact our . Use the Client Log In at the top of the screen to access the Client Services program.

Click on Managed Care from the right side of this page to be connected to our Managed Care Network system.
OSHA:
In the event of death, catastrophic injury, amputation or an accident that involved 3 or more persons you must contact OSHA (800) 321-6742 or in California, CALOSHA (877) 922-7233. NOTE: Some states have different requirements. Contact your appropriate state agency for additional reportable claims.
