Report a Claim

There are 3 ways to report claims to Broadspire: via Internet, E-Mail, or Telephone.

Internet: TelePlus Claims Capture: In order to access the TelePlus system, you will need a login ID and Password. Contact our Smyrna office if you do not have one already. For training on TelePlus, click on the following PowerPoint presentation for insturctions.
E-Mail: Fill out the claims form and send it to

Be sure that you have the following information ready when you report an injury:

The six digit location site code (the last 6 digits of the certificate of insurance for the location where the injury occurred.
Name of the worksite location where the injury occurred
Date, Time, and Place of accident
Name and Social Security number of injured worker
NCCI Code of the injured worker
Description of the accident and if a 3rd party was involved
Was this a catastrophic accident or did a death occur
Were any authorities contacted or involved

Reminder: In the event of death, catastrophic injury, amputation or an accident that involved 3 or more persons you must contact OSHA (800) 321-6742 or in California, CALOSHA (877) 922-7233. NOTE: Some states have different requirements. Contact your appropriate state agency for additional reportable claims.

Once the claim has been reported you will receive a copy via fax within 24 hours.
